Algorithm of actions that will help to eliminate problems with the generation of COVID-certificate
You received the status "No data about you" after requesting to create a document
If you requested the Diia mobile application, try to repeat it through the Diia portal - https://diia.gov.ua. If the portal still replies that "No data about you", contact your family doctor or the doctor who vaccinated you. Ask to check your data:
Full name in Ukrainian and English, as in passport documents.
RNOTAC (registration number of the taxpayer's account card, formerly called the individual tax number - TIN).
URN is a unique record number that is entered about each citizen of Ukraine in the Unified Demographic Register and helps to make digital identification of a person.
Date of birth, as in the passport/birth certificate.
Current phone number where you can receive and send SMS.
If you have a child under the age of 18, make sure that his or her, not yours, RNOTAC and URN are entered in the patient's card.
Also, ask your doctor to check if there are any duplicates of your profile in the system or a match with another person's profile.
You received the status "Certificate generation error" after requesting a COVID certificate
It is recommended that you contact your family doctor or the doctor who vaccinated you with a request to verify the data described above.
You received the status "Vaccination certificate is not available"
If you have received 1 or 2 doses of vaccination, please contact the vaccination point to have your data entered. It is recommended to update the mobile application, then send a request for the generation of a COVID certificate again or do it through the Diia portal - https://diia.gov.ua/.
You have received the status "Vaccination certificate is not available" with the following text "You were not vaccinated in Ukraine or the vaccination data was entered by the wrong doctor who made the vaccination"
If you were vaccinated against COVID-19 outside of Ukraine, it is not possible to generate a COVID certificate that would take into account the vaccination you have received. The Ministry of Health is currently working on this possibility.
You have received the status: "Vaccine batch number is incorrect"
You need to contact the doctor who performed the vaccination and entered the medical record of vaccination into the electronic healthcare system with a request to mark the entered data as erroneous and add the correct data again, without error. Without such action, you will not be able to receive an International Vaccination Certificate and will not receive a COVID certificate.
You have received the status: "Unfortunately, an error occurred".
